Questionaires follow a fairly simple format, for example :

Only one answer field is allowed on each line, and each 'Question' line
must be immediately *preceded* by a template such as those shown above
to indicate the maximum allowable length of the answer.
Often you will not need to include the name question, since GT will
automatically stamp the answers with the callers name as used at log on
time.
A special field is available to include the date and time in the answer
file :

Limits:
Up to 50 questions per questionaire
Individual answer fields can be up to 80 characters long
In practice, questionaires are best kept brief :
a) It is not possible to go back and change the answer to a
previous question.
b) Callers frequently have difficulty answering questionaires on
noisy lines - no opportunity to repeat the question, or to
change the answers to a previous question.
c) In many cases it makes more sense to invite the caller to
construct a reply offline and to upload the answer in the form
of a message.
